The need for Adult Day Health Center was recognized for the community where seniors and other individuals can benefit from a nurturing environment with social stimulation which can reduce the symptoms and progression of mental illness.

Enrollment 

 

Once prospective participants fill out the initial paperwork, we sit with them and tailor a program based on their need and preference.  Our Inter-Disciplinary Team makes an assessment and gives its recommendations for what would maximize those needs. The interdisciplinary team will address the participants’ family’s needs, to provide respite to family and caregivers. It is important that the participants not only engage in the many activities the center offers, but that they also be given help in coordinating with their family and spouse. La Casa's social workers and psychologists will consistently bridge the needs of the families in a way that compliments the needs of the participants. An explicit goal of the center is to liaison with the families and the community to maximize benefit and impact.

Clients potentially eligible for ADHC Center Services include: 

 

Seniors Adults with physical deficits that require health or medical monitoring & supervision;  Adults who have few or inadequate support systems and need assistance with activities of daily living, 

Adults in transition from hospitalization to independent living, Adults with developmental Disabilities, Adults in recovery from strokes or other physical traumas, Adults with memory loss and cognitive impairment that interfere with daily functioning, Adults with mental deficits such as depression or emotional problems that interfere with their ability to cope on a daily basis and Adults in need of daily structured activities and assistance with everyday living skills.
